HIV Care Coordination
& Women and Children's Programs

The HIV/AIDS Care Coordination and Women & Children’s Program is committed to assisting HIV/AIDS clients in accessing services and programs to further their well - being. Services are offered to clients in East Baton Rouge, Livingston, West Baton Rouge, Iberville, Ascension, Point Coupee, and East and West Feliciana parishes.  The following programs are available to eligible HIV positive clients.

               Ryan White CARE ACT Title II HIV/AIDS 
                       Services for Men and Women

Care Coordination - service that links clients to health care and psycho-social assistance. Care coordination includes structured access to appropriate levels of medical and support services, insuring continuity of care and ongoing assessment of client.  

Rent/Mortgage or Utilities Payment Assistance – We can refer clients to another agency which can provide Housing Opportunities for Persons with AIDS (HOPWA). This short-term assistance (times a year or less per client year) that can assist with rent payment, mortgage payments or utilities.  We refer clients who need this assistance to the Office of Social Services.

Medication Assistance – Provides assistance in obtaining medications that are not on the Louisiana Drug Assistance Program (ADAP) of the Louisiana Public Hospital’s Drug Formulary. This program includes provisions for direct nutrition supplements.

Dental Assistance – designed to provide financial assistance for diagnostic, prophylactic, and therapeutic services.

Mental Health/Supportive Counseling-  Services include individual and family counseling,and support groups.

Referrals to housing programs - There are several housing programs for our clients.  The case manager refers appropriate clients to these housing programs run by community based organizations. 

Transportation – offered in the form of bus cards, taxis, bus tickets, lift tickets and gas reimbursement.
